Please bring your completed paperwork with you to your first visit. This will help expedite your orthodontic evaluation. You initial consultation will last approximately one hour. During your first visit, we will take digital photographs of your face and teeth, and discuss a diagnosis and treatment options. Digital radiographs may be taken, if necessary, as part of your intial exam.
The following form are included in the initial welcome packet. In the event that you have not received them, please print and complete the appropriate forms and bring it to your appointment.
